บทคัดย่อ
การวิจัยเรื่อง “การมีส่วนร่วมของประชาชนต่อการดำเนินงานด้านสุขภาพท้องถิ่นของชุมชนบ้านบางแก้ว เทศบาลเมืองบางแก้ว จังหวัดสมุทรปราการ” มีวัตถุประสงค์ 1) เพื่อศึกษาการมีส่วนร่วมของประชาชนต่อการดำเนินงานด้านสุขภาพท้องถิ่นของชุมชนบ้านบางแก้ว เทศบาลเมืองบางแก้ว จังหวัดสมุทรปราการ และ 2) เพื่อเสนอแนะแนวทางส่งเสริมการมีส่วนร่วมของประชาชนต่อการดำเนินงานด้านสุขภาพท้องถิ่นของชุมชนบ้านบางแก้ว เทศบาลเมืองบางแก้ว จังหวัดสมุทรปราการ การวิจัยครั้งนี้เป็นการวิจัยเชิงคุณภาพ โดยใช้การสัมภาษณ์เชิงลึก (In-depth Interview) และการสังเกตการณ์แบบไม่มีส่วนร่วม (Non-participant Observation) เป็นเครื่องมือในการเก็บรวบรวมข้อมูล ผู้ให้ข้อมูลสำคัญประกอบด้วยเจ้าหน้าที่ผู้ปฏิบัติงานที่เกี่ยวข้องกับการดำเนินงานด้านสุขภาพท้องถิ่นและหน่วยงานด้านสาธารณสุขในพื้นที่ ตลอดจนผู้นำชุมชนและประชาชนที่มีประสบการณ์ในการเข้าร่วมกิจกรรมด้านสุขภาพในพื้นที่ ซึ่งได้รับการคัดเลือกโดยวิธีการเลือกแบบเฉพาะเจาะจง (Purposive Sampling) และขยายกลุ่มผู้ให้ข้อมูลด้วยวิธีการเลือกแบบลูกโซ่ (Snowball Sampling) ข้อมูลที่ได้นำมาวิเคราะห์เชิงเนื้อหา (Content Analysis) และตีความตามกรอบแนวคิดการมีส่วนร่วมของประชาชน
ผลการศึกษาพบว่า การมีส่วนร่วมของประชาชนต่อการดำเนินงานด้านสุขภาพท้องถิ่นของชุมชนบ้านบางแก้วมีลักษณะเป็นการมีส่วนร่วมเชิงปฏิบัติการ โดยประชาชนมีบทบาทในฐานะผู้เข้าร่วมกิจกรรม ผู้ให้ความร่วมมือ และผู้สนับสนุนการดำเนินงานด้านสุขภาพของหน่วยงานภาครัฐผ่านกลไกผู้นำชุมชนและอาสาสมัครสาธารณสุขประจำหมู่บ้านเป็นสำคัญ ในภาวะปกติ การมีส่วนร่วมส่วนใหญ่อยู่ในลักษณะการตอบสนองต่อกิจกรรมหรือโครงการที่หน่วยงานภาครัฐกำหนด ขณะที่ในภาวะวิกฤต โดยเฉพาะสถานการณ์การแพร่ระบาดของโรคโควิด-19 ประชาชนมีบทบาทเชิงรุกมากขึ้น ทั้งในด้านการเฝ้าระวัง การสื่อสารข้อมูล การประสานงาน และการช่วยเหลือประชาชนในระดับชุมชน อย่างไรก็ตาม การมีส่วนร่วมยังคงมีข้อจำกัดในระดับการกำหนดนโยบายและการตัดสินใจเชิงยุทธศาสตร์ ซึ่งยังอยู่ภายใต้กรอบการบริหารของหน่วยงานภาครัฐ
นอกจากนี้ ผลการศึกษาพบว่า ปัจจัยที่มีผลต่อระดับและความเข้มข้นของการมีส่วนร่วมของประชาชน ประกอบด้วย โครงสร้างการบริหารของภาครัฐ บทบาทของผู้นำชุมชนและเครือข่ายทางสังคม ตลอดจนบริบทของสถานการณ์และระดับความเร่งด่วนของปัญหา โดยเฉพาะอย่างยิ่งความไว้วางใจ ความใกล้ชิด และทุนทางสังคมภายในชุมชน ซึ่งเป็นกลไกสำคัญที่ช่วยส่งเสริมความร่วมมือระหว่างประชาชนกับหน่วยงานภาครัฐในระดับพื้นที่
ดังนั้น หน่วยงานภาครัฐและองค์กรปกครองส่วนท้องถิ่นควรส่งเสริมกลไกการมีส่วนร่วมของประชาชนให้ครอบคลุมมากกว่าระดับการปฏิบัติการ เปิดพื้นที่ให้ประชาชนสามารถสะท้อนปัญหา แสดงความคิดเห็น และมีส่วนร่วมในการกำหนดแนวทางการดำเนินงานด้านสุขภาพท้องถิ่นได้อย่างต่อเนื่อง อันจะนำไปสู่การพัฒนาระบบสุขภาพท้องถิ่นที่มีประสิทธิภาพ สอดคล้องกับบริบทของชุมชน และเกิดความยั่งยืนในระยะยาว

Abstract
This research aimed to: (1) examine public participation in local health operations in Ban Bang Kaeo Community, Bang Kaeo Municipality, Samut Prakan Province; and (2) propose guidelines for enhancing public participation in local health operations in the community.
This study employed a qualitative research approach. Data were collected through in-depth interviews and non-participant observation. Key informants consisted of local government officials responsible for local health operations and public health services, as well as community leaders and local residents who had experience participating in community health activities. Participants were selected using purposive sampling, and additional informants were recruited through snowball sampling. The collected data were analyzed using content analysis and interpreted based on the conceptual framework of public participation.
The findings revealed that public participation in local health operations in the Ban Bang Kaeo community was predominantly operational in nature. Community members mainly participated as activity participants, collaborators, and supporters of health initiatives organized by public agencies, primarily through community leaders and village health volunteers. Under normal circumstances, public participation was largely characterized by responses to activities or projects initiated by government agencies. However, during crisis situations, particularly the COVID-19 pandemic, community members assumed more proactive roles in disease surveillance, health communication, coordination, and community assistance. Nevertheless, public participation remained limited in terms of policy formulation and strategic decision-making, which continued to be governed primarily by public sector administrative structures.
The study further found that the level and intensity of public participation were influenced by several factors, including the administrative structure of government agencies, the roles of community leaders and social networks, and the contextual conditions and urgency of local problems. In particular, trust, social connectedness, and social capital within the community were identified as key mechanisms facilitating collaboration between local residents and public agencies.
Based on these findings, government agencies and local administrative organizations should strengthen mechanisms for public participation beyond the operational level by creating continuous opportunities for citizens to express their concerns, share opinions, and participate in decision-making regarding local health operations. Such efforts would contribute to the development of an effective local health system that is responsive to community contexts and sustainable in the long term.
