บทคัดย่อ
บทนำ: ตัวบ่งชี้ทางชีวภาพ (Biological Indicators; BIs) เป็นเครื่องมือสำคัญในการทวนสอบประสิทธิผลของกระบวนการทำให้ปราศจากเชื้อด้วยไอน้ำ โดยเฉพาะในหน่วยเวชภัณฑ์ปลอดเชื้อที่ต้องการความถูกต้องและความรวดเร็วในการยืนยันผลของรอบการทำให้ปราศจากเชื้อ
วัตถุประสงค์: การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิผลของตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วมากภายใน 5 นาที กับตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วภายใน 24 นาที ในกระบวนการทำให้ปราศจากเชื้อด้วยไอน้ำ ระบบปั๊มสูญญากาศที่อุณหภูมิ 134 °C ในหน่วยเวชภัณฑ์ปลอดเชื้อ โรงพยาบาลศูนย์การแพทย์มหาวิทยาลัยแม่ฟ้าหลวง
วิธีการศึกษา: การศึกษานี้เป็นการศึกษาเชิงทดลองเปรียบเทียบ โดยทดสอบตัวบ่งชี้ทางชีวภาพ 2 ชนิด ได้แก่ ยี่ห้อ A ซึ่งเป็นตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วมากภายใน 5 นาที และยี่ห้อ B ซึ่งเป็นตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วภายใน 24 นาที รวมจำนวน 600 ตัวอย่าง การทดลองแบ่งตามระยะเวลาการสัมผัสไอน้ำ 3 ระดับ ได้แก่ 1 นาที 3 นาที และ 5 นาที เพื่อจำลองสภาวะการทำให้ปราศจากเชื้อที่ไม่สมบูรณ์ ก้ำกึ่ง และสมบูรณ์ ตามลำดับ วิเคราะห์ผลโดยเปรียบเทียบสัดส่วนผลบวกและลบระหว่างผลิตภัณฑ์ทั้งสองชนิดในแต่ละช่วงเวลา และกำหนดระดับนัยสำคัญทางสถิติที่ p < 0.05
ผลการศึกษา: ในรอบ 1 นาที ซึ่งเป็นสภาวะการทำให้ปราศจากเชื้อที่ไม่สมบูรณ์ ตัวบ่งชี้ทางชีวภาพทั้งสองชนิดให้ผลบวกครบถ้วน 100.00% ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติระหว่างยี่ห้อ A และยี่ห้อ B ในรอบ 3 นาที ซึ่งเป็นสภาวะก้ำกึ่งและเป็นจุดสำคัญในการประเมินความไวของตัวบ่งชี้ทางชีวภาพ พบว่ายี่ห้อ A ให้ผลลบ 95.00% ขณะที่ยี่ห้อ B ให้ผลลบ 85.00% แสดงถึงความแตกต่างของความไว 10% และแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p = 0.032) ในรอบ 5 นาที ซึ่งเป็นสภาวะการทำให้ปราศจากเชื้อที่สมบูรณ์ ตัวบ่งชี้ทางชีวภาพทั้งสองชนิดให้ผลลบครบถ้วน 100.00% สะท้อนความจำเพาะของผลการทดสอบภายใต้เงื่อนไขการทดลองนี้
บทสรุป: ตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วมากภายใน 5 นาทีมีความไวสูงกว่าตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วภายใน 24 นาทีอย่างมีนัยสำคัญทางสถิติในรอบก้ำกึ่งที่ 3 นาที ซึ่งเป็นสภาวะสำคัญต่อการตรวจจับความล้มเหลวของกระบวนการทำให้ปราศจากเชื้อ ผลการศึกษาสนับสนุนว่าตัวบ่งชี้ทางชีวภาพแบบอ่านผลเร็วมากอาจมีความเหมาะสมสำหรับการทวนสอบคุณภาพแบบรายโหลดในบริบทที่ต้องการผลตรวจที่รวดเร็วและมีความไวสูง ทั้งนี้ ประเด็นด้านภาระงาน ต้นทุน และผลกระทบด้านอาชีวอนามัยควรได้รับการศึกษาเพิ่มเติมในอนาคต

Abstract
Background: Biological indicators (BIs) are essential tools for verifying the effectiveness of steam sterilization processes, particularly in Central Sterile Supply Departments, where accurate and timely confirmation of sterilization cycles is required.
Objective: This study aimed to compare the effectiveness of a 5-minute super rapid-readout biological indicator and a 24-minute rapid-readout biological indicator during pre-vacuum steam sterilization at 134 °C in the Central Sterile Supply Department of Mae Fah Luang University Medical Center Hospital.
Methods: This comparative experimental study evaluated two types of biological indicators: Brand A, a 5-minute super rapid-readout BI, and Brand B, a 24-minute rapid-readout BI. A total of 600 BI samples were tested. The experiment was conducted under three steam exposure durations: 1, 3, and 5 minutes, representing suboptimal, marginal, and adequate sterilization conditions, respectively. The proportions of positive and negative results between the two products were compared at each exposure duration. Statistical significance was set at p < 0.05.
Results: In the 1-minute cycle, representing a suboptimal sterilization condition, both biological indicators yielded 100.00% positive results, with no statistically significant difference between Brand A and Brand B. In the 3-minute cycle, representing a marginal condition and a critical point for assessing BI sensitivity, Brand A yielded 95.00% positive results, whereas Brand B yielded 85.00%. This indicated a 10-percentage-point difference in sensitivity, which was statistically significant (p = 0.032). In the 5-minute cycle, which represents an adequate sterilization condition, both biological indicators yielded 100.00% negative results, indicating specificity under the experimental conditions.
Conclusion: The 5-minute super rapid-readout biological indicator demonstrated significantly higher sensitivity than the 24-minute rapid-readout biological indicator in the 3-minute marginal cycle, which is a critical condition for detecting steam sterilization process failure. These findings support the potential suitability of super-rapid-readout BIs for load-by-load quality monitoring in settings that require rapid, highly sensitive verification. However, further studies are needed to evaluate workload, cost, and occupational health implications.
