บทคัดย่อ

ภาวะภูมิไวเกิน (hypersensitivity reactions; HSRs) จากแพคลิแทกเซล มีอุบัติการณ์ร้อยละ 5–30 และมักเกิดขึ้นในรอบการรักษาที่ 1–2 กลไกหลักเป็นแบบ non-IgE-mediated ผ่านกระบวนการ complement activation-related pseudoallergy โดยมีสารช่วยในตำรับ Cremophor EL เป็นปัจจัยกระตุ้นที่สำคัญ
บทความปริทัศน์นี้สรุปพยาธิสรีรวิทยา ลักษณะทางคลินิก การประเมินความรุนแรงตามเกณฑ์ Common Terminology Criteria for Adverse Events และแนวทางการจัดการในเวชปฏิบัติ ครอบคลุมการให้ยาป้องกันก่อนการรักษา การจัดการภาวะฉุกเฉิน การให้ยาซ้ำ และการทำให้ผู้ป่วยทนต่อยาแบบเร่งด่วน
นอกจากนี้ บทความยังสังเคราะห์แนวทางการดูแลผู้ป่วยหลังเกิด HSRs ตั้งแต่การประเมินความรุนแรง การพิจารณาให้ยาซ้ำ (re-challenge) หรือการเปลี่ยนสูตรการรักษาให้เหมาะสมกับผู้ป่วยแต่ละราย ทั้งนี้ การเลือกแนวทางการรักษาควรพิจารณาร่วมกันจากระดับความรุนแรงของอาการ หลักฐานเชิงประจักษ์ และบริบทด้านทรัพยากรของสถานพยาบาล เพื่อยกระดับความปลอดภัยและความต่อเนื่องในการรักษาผู้ป่วย

Abstract

Abstract: Paclitaxel-induced hypersensitivity reactions (HSRs) have an incidence of 5%–30% and most commonly occur during the first or second treatment cycle. The predominant mechanism is non-IgE-mediated and involves complement activation–related pseudoallergy, with Cremophor EL, a formulation excipient, serving as a major triggering factor. This review summarizes the pathophys-iology, clinical manifestations, Common Terminology Criteria for Adverse Events – based severity assessment, and practical management strategies for paclitaxel-induced HSRs, covering premedica-tion, acute reaction management, re-challenge, and rapid drug desensitization. It also synthesizes practical clinical management strategies following HSRs, including severity assessment, selection of re-challenge strategies, and treatment modification when appropriate. Clinical decision-making should integrate reaction severity, available evidence, and institutional resources to optimize patient safety and treatment continuity.
